A signed affirmation confirming that the information provided for each case is accurate to the best of the representative’s knowledge. Download a sample affirmation in the Rules, Forms, and Fees Library.

Demand for Arbitration or Request for Mediation

A separate Demand for Arbitration or Request for Mediation must be filed for each case, with complete contact information for all parties and representatives. Ensure each document is saved with a unique identifier that corresponds to the case-filing spreadsheet. For example, if the unique identifier on the spreadsheet is 1, the demand should be named 1.pdf.
